My main concern is that there's no exit hole..? Highly unlikely, even though the entry hole looks like it's at a downward angle

No 2nd hole is your main concern as in you think it might be fake? Looks good to me. Maybe just hard to say for sure under what circumstances it ended up like it did.

One thought though, if this was on the guy's head when damaged one would expect much more blood damage to the liner.

Its good, but don't think its a KIA. It looks more like shrapnel to me too.

If it was a KIA I would expect the liner to be seriously blood stained...
